The 2008 Mack Johnson Memorial Golf Classic scholarship fundraising event will be held at Kingwood Country Club on Tuesday, April 8th.

The playing field is limited to 160 players.  

Using the electronic forms on this page, interested parties can register to play, sponsor, volunteer or only attend the awards dinner following the 2008 Mack Johnson Memorial Golf Classic.

NOTE:  Player, Sponsor and Awards Dinner-Only Seats are based on a first-come, first-served basis as to receipt of both registration and payment. All payments should be received prior to the golf date on April 4th.

Player Registration Form

All players must register using the Player Registration form. This form provides for both individual and group player registration; register only yourself, or as many as 12 players.

NOTE: The deadline for player registration and payment of player registration fee(s) is April 1st.  All players must register and pay by this date.

Submitting a completed Player Registration form will generate a confirmation screen that presents instructions on how to make payment, either online or by check. Players must register and pay in order to be assured of a slot in this tournament. Failure to make payment promptly may result in your party being shut out of the tournament.

Sponsor Registration Form

All sponsors must register using the Sponsor Registration form. This form provides for selection of a variety of sponsorships including:

A "zero" in the "Available" column, above, indicates a sponsorship that is sold out.

Premium Sponsor Benefits
PLATINUM, GOLD and SILVER

Premium sponsors will receive company recognition before, during and after the golf tournament.

Monies received from PLATINUM, GOLD and SILVER premium sponsors will be applied  to Scholarships bearing the names of these sponsors.

Platinum and Gold sponsors will have their names featured on a display banner and will receive a complimentary hole sponsorship and a complimentary player foursome.

Following the tournament, ITMA will run a full-page ad in Gulf Shipper Magazine to recognize and thank all premium sponsors for their generous support of ITMA's scholarship program.

Volunteer Registration Form

Successful execution of the Mack Johnson golf fundraising event is due largely to the dedication and commitment of volunteers.

Who is responsible for an orderly check-in process?  Who is responsible for manning the beverage stations and beverage carts? Who makes sure the weather is perfect? And who makes sure that every player shoots under 80?  Volunteers!

If you are interested in volunteering (and subsequently basking in the praise and admiration of fellow members), please register using the Volunteer Registration form. The volunteer registration form provides for selection of various volunteer categories, as follows:

Occasionally (but not every year), there is an abundance of volunteers. In this event, volunteer assignments may be aligned differently than as requested.

Awards Dinner-Only Registration Form

Persons not able to attend the golf tournament but wishing to attend the awards dinner following the tournament must register using the Award Dinner-Only Registration form. This form provides for both member and guest registration.

The cost to attend just the awards dinner is $25 per member, and $30 per non member.

NOTE: The deadline for awards dinner registration and payment of fee(s) is Friday, April 4th.  All attendees must register and pay by this date.

Submitting a completed Awards Dinner Registration form will generate a confirmation screen that presents instructions on how to make payment, either online or by check. Attendees must register and pay in order to be assured of a seat at the awards dinner. Failure to make payment promptly may result in your party being shut out of the awards dinner.
